Have you been diagnosed with atopic dermatitis (also called eczema) for at least 12 months?

Does your atopic dermatitis currently cover at least 10% of your skin (10% is slightly more than the skin on one arm)?

Have you used topical medications (steroid creams or similar) for atopic dermatitis in the past 6 months that were not entirely satisfactory?

Have you had breast cancer in the last 10 years?

Have you had any serious or life-threatening infections in the past 6 months?

Do you or someone you are related to have any inflammatory bowel diseases such as Crohn’s disease or ulcerative colitis?
